Jennifer Scott, MSW, CSW

Jennifer Scott, MSW, CSW, is a board-certified social worker caring for patients of all ages. She brings nearly two decades of experience as a case manager and is certified in sand tray therapy. As part of her practice, she provides targeted case management services in behavioral health clinics and emergency departments, comprehensive assessments, care coordination and resource linkage to support her patients. She also collaborates with multidisciplinary teams to develop and implement individualized care plans, ensuring continuity of services and access to community resources. Jennifer is a helper at heart. She felt a calling on her life to provide compassion, hope and love to those in need. She has a unique ability to connect with patients and their families to provide specialized support. Jennifer earned her bachelor’s degree in interdisciplinary early childhood education from Eastern Kentucky University, where she then completed her master's in social work. Jennifer is certified through the Kentucky Board of Social Work.

In her free time, Jennifer enjoys spending time with her husband and two children, cooking and baking.
