Benjamin Ensell, PA-C, is a dedicated and compassionate licensed physician assistant with over 10 years of experience in primary care. He earned his Master of Physician Assistant Studies from Bethel University in McKenzie, TN, and his Bachelor of Science in Pre-Medicine from the same institution. Benjamin has worked as a physician assistant at BayCare Medical Group in Tampa, FL, Brevard Health Alliance in Melbourne and Titusville, FL, and Quality Medical Care in Melbourne, FL He served his country as a United States Army cavalry scout and his platoon’s combat lifesaverbased in Fort Steward, GA. He is committed to patient-centered, whole-person and collaborative health care.

Dianet Falcon, DO, is a family medicine physician who obtained her medical degree from Edward Via College of Osteopathic Medicine in Auburn, AL. She completed a residency in Family Medicine in Daytona Beach, FL. In 2019, Dr. Falcon was the recipient of the William G. Anderson Minority Award from the American Osteopathic Foundation for her work with the Florida Hispanic community. She began her medical career at AdventHealth East Orlando, where she was an emergency department scribe. Fluent in English and Spanish, Dr. Falcon is involved with diversity, equity and inclusion initiatives and works to make medical care more accessible for Spanish speakers. Dr. Falcon is dedicated to providing the best care for each patient’s body, mind and spirit.

Emily R. Green, PA-C, is a highly motivated Certified-Physician Assistant with extensive experience across diverse clinical settings. She earned both her Master of Science in Physician Assistant Studies and Bachelor of Science in Physician Assistant Studies from the Rochester Institute of Technology. Emily is deeply committed to empowering patients to achieve their wellness goals by delivering consistent, high-quality, whole-person care.

Kate M. Loftis, RN, BSN, MMS, PA-C, is an experienced Physician Assistant with a diverse background in urgent care, primary care, and specialty care. She earned her Master of Medical Science in Physician Assistant Studies from Nova Southeastern University, graduating with highest honors, and her Bachelor of Science in Nursing from Florida State University, magna cum laude. Kate currently serves as the APP Director for AdventHealth’s North Division and has held leadership roles such as Chair of the Advanced Practice Provider Council. Kate is passionate about mentoring future healthcare providers and has served as a preceptor at Florida State University. She is committed to providing exceptional care at every step and lives up to AdventHealth’s promise of wholeness, restoration and health in all her work.

Haroldo D. Melo, MD is a board certified Internal Medicine physician with more than 12 years of experience in private practice. He received his medical degree from Universidad de Monterrey in Monterrey, Mexico and completed his residency at Metrowest Medical Center in Framingham, Massachusetts.

Adam Pasternak, DO, CAQSM, is a board-certified family medicine physician with a certificate of added qualification in sports medicine. After graduating magna cum laude from Missouri State University, Dr. Pasternak received his medical degree from the Lake Erie College of Osteopathic Medicine – Bradenton. He went on to complete an internship at St. Luke’s Des Peres Hospital, a family medicine residency at Saint Louis University, and a primary care sports medicine fellowship at the Kirk Kerkorian School of Medicine at UNLV.

Prior to joining AdventHealth Medical Group, Dr. Pasternak previously served as an urgent care physician at AdventHealth Centra Care in Daytona, urgent care physician at Mercy Hospital in St. Louis and worked with Mercy Clinic Sports Medicine, where he was a physician for high school, college, and professional sports and covered athletic events around the St. Louis area. Dr. Pasternak will be seeing patients from children to geriatric for management of acute and chronic disease, well visits, sick visits, non-operative musculoskeletal injuries, joint injections, immunizations, and other primary care needs.

Sarah Sakala, DNP, APRN, FNP-BC, is a certified family nurse practitioner. After graduating with a Bachelor of Science in Nursing from the University of Central Florida, Sarah went on to earn her Doctor of Nursing Practice degree. A highly experienced clinician, Sarah has extensive experience caring for patients of all ages. Her clinical interests include pediatrics, cardiology, primary care and autism spectrum disorders.
